  4. Premature discharge with “flu-like” labels that are later declared as sepsis.

  • CMS: SEP-1 is moving from “reporting” to “pay-for-performance.”
    CMS finalized inclusion of the Severe Sepsis/Septic Shock (SEP-1) bundle in the Hospital Value-Based Purchasing (VBP) program beginning with the FY 2026 program year (baseline 2023; performance period 2025). Practically, that means hospitals’ payment adjustments will reflect SEP-1 performance -placing additional weight behind timely lactate testing, blood cultures before antibiotics, early antibiotics, fluids, vasopressors (as indicated), and reassessment. What Typically Decides These Cases

When I review sepsis files, I look for four things that often determine defensibility:

  1. Recognition & Time Zero: Did the team document the moment sepsis/shock was sufficiently suspected, and did actions follow promptly?
  2. Antibiotics & Fluids: Were orders placed/administered expeditiously with rationale documented (especially if choosing to observe first)?
  3. Structured Pathway: Did the ED use a standardized sepsis pathway (screening, lactate, blood cultures before antibiotics, repeat lactate, reassessment)?
  4. Systems Support: Did the hospital’s policies, staffing, pharmacy turnaround, and EHR prompts enable timely care (not merely expect it)?
